Success Home Care began in 2017 with a simple goal: to provide seniors with high-quality care in the comfort of their own homes, delivered by people they can trust. What started as a small team with a big vision has grown into a full-service home care company serving clients and families across our community every day.
Success Home Care was founded by Bo Jia, who brings over 20 years of experience in home care. What began with a strong commitment to hands-on, personal support has grown into a collaborative team effort. Guided by the clinical leadership of our RN Candice, who has a people-first approach that supports the team to provide consistency, compassion, and professionalism.
We offer flexible, personalized in-home care that meets people where they’re at. Whether it's daily personal care, companionship, light housekeeping, or just a few hours of weekly support, our goal is to help clients feel safe and comfortable at home.
Each care plan is developed in partnership with our clients and their families, and every caregiver is thoughtfully chosen to match each person’s needs. Our approach is personal and involved. When you call, you speak with someone who knows your name. When things change, we work with you to adjust. We care deeply about the little details that help people feel truly supported.

Care Coordinator
Candice brings over a decade of nursing experience and a deep sense of compassion to her role. As our clinical lead, she conducts in-home assessments, develops personalized care plans, and provides ongoing supervision to ensure every client receives safe, high-quality care. Candice’s approach is hands-on, holistic, and heart-led—always focused on empowering clients and supporting caregivers with confidence and clarity.
Bo Jia
Founder & CEO
With over 20 years of experience in the home care sector, Bo founded Success Solutions Home Health in 2017 to meet a growing need for more reliable, compassionate in-home support. His vision was simple: give families peace of mind and give seniors the ability to age at home with dignity. Bo brings entrepreneurial insight, industry knowledge, and a deep respect for caregiving to everything we do. He continues to guide the company’s growth while staying grounded in the values that started it all—trust, integrity, and human connection.
